Goals Set by the Person
We ask the people we support what they want to achieve — not what we think they should want. Goals might be as simple as getting dressed independently each morning, or as significant as attending a community group. Every goal is valid.
Coordinated with Your Care Team
We work alongside community mental health teams, GPs, care coordinators, social workers, and family members. We share relevant observations, flag concerns, and ensure that everyone involved is informed and working together.
